I see this almost every day in my clinic.
A patient walks in — or rather, limps in — holding X-rays. After I examine them, I realise they have been in serious pain for two or three years already. The knee is badly damaged. And when I ask why they waited so long, I always hear the same things.
“I thought the pain would go away on its own.”
“I was scared of the operation.”
“My family said wait a little longer.”
I understand. These are natural fears. But after doing more than 6,000 joint replacement surgeries, I can tell you one thing clearly — waiting almost never helps. In most cases, it makes things worse.
What Happens When You Keep Waiting
When your knee is damaged and you keep walking on it, the muscles around it start getting weak. Your body automatically shifts weight to protect the bad knee. This puts extra pressure on your other knee, your hip, and your lower back.
After some time, it is not just one joint that has a problem — it is three or four.
Patients who delay surgery too long also take much longer to recover after the operation. They come in with weak muscles, stiff joints, and sometimes with bones that have become crooked. The surgery works, but the body takes longer to come back to normal.
The Real Reason People Delay — Fear
Most people are not delaying because of a medical reason. They are delaying because they are scared.
Scared of anaesthesia. Scared of the hospital. Scared the operation will not work. Scared they will be bedridden for months.
These fears come from old information. Knee replacement surgery has changed a lot in the last ten years. With robotic systems like RoboLens FT3D, the implant is placed with very high accuracy. Recovery is faster. Complications are much less common than before.
Almost every patient I have operated on tells me afterward — “Doctor, I wish I had come earlier.”
How Do You Know It Is Time?
There is no single rule for everyone. But here are some clear signs that it is time to speak to an orthopaedic surgeon:
- Knee pain wakes you up at night
- You have stopped doing things you used to enjoy — walking in the park, visiting family, going to the market
- Painkillers are working less and less
- Your knee swells up regularly
- You feel a grinding feeling inside the knee when you walk
- Walking even 200 to 300 metres is painful
If these things have been happening for more than a year and physiotherapy has not helped much, it is time for a proper conversation with a specialist.
Pain Affects More Than Just the Body
Living with knee pain for years does something to a person mentally as well. Patients stop going out. They miss weddings, family functions, their grandchildren’s school events. They stop exercising, which leads to weight gain, which makes the knee worse.
After surgery, I have seen patients who had not stepped outside their house in months suddenly start living their normal life again. That is not just a physical change — that is quality of life coming back.
Do Not Wait Too Long
There is a point where waiting creates real problems for the surgery. If the bone has become very deformed, or if the muscles are very weak, the operation becomes more complicated. Recovery also becomes harder.
My simple advice: do not let the fear of a conversation stop you from having it. Come in, get your knee checked properly, and find out what your options are. The decision is always yours — but you need the right information to make it.

Frequently Asked Questions
Is it safe to delay knee replacement surgery?
Delaying knee replacement when it is clinically indicated can lead to progressive joint damage, muscle weakness, and worsening deformity that makes surgery more complex. It can also reduce the quality of recovery. Dr TS Gill recommends evaluation early to make an informed decision rather than waiting until pain becomes unbearable.
What happens to the knee if you wait too long for knee replacement?
Prolonged delay can lead to severe deformity (bow-leg or knock-knee), loss of surrounding muscle, and bone erosion that may require more complex revision-type surgery. Early surgical correction generally produces better functional outcomes.
Is knee replacement surgery risky?
Knee replacement is one of the most commonly performed and well-studied orthopaedic procedures. Serious complications are uncommon. Robotic knee replacement, as performed by Dr TS Gill, further reduces the risk of malalignment and implant-related complications.
How do I overcome fear of knee replacement surgery?
Understanding the procedure, recovery timeline, and success rates helps reduce fear. Dr TS Gill takes time to explain the surgery in detail, answer all questions, and ensure patients feel confident before proceeding. Patients are also connected with others who have recovered well.
